Department of Public Health (DPH) Iowa Healthy Communities Initiative Grant Program This program offers Community Wellness Grants and Harkin Wellness Grants for Local Health Improvement initiatives. Eligible applicants may seek funds for coordinated programs at the community level to encourage healthier lifestyles, promote wellness and prevent the health and financial consequences of chronic disease that are often lifestyle related. These grants can also fund infrastructure (such as trails, etc) if amenities are connected to community health program with measurable impacts.
